Privacy Policy.
What we collect, why we collect it, and what you can ask us to do with it.

Who we are
This website is run by Love at Home CDS & In-Home Services ("Love at Home," "we," or "us"). We are a non-medical in-home care agency at 677 N New Ballas Road, Suite 214, Creve Coeur, MO 63141. We serve families across the St. Louis metro in Missouri through Medicaid, Consumer Directed Services, Healthy Children and Youth, veterans support, and private pay, with nurse visits where a care plan calls for them.
This policy explains what information we collect through https://loveathomehealth.com, how we use it, who we share it with, and the choices you have. It covers this website and its two forms: the free in-home assessment request and the caregiver job application.
What we collect through the assessment form
The free in-home assessment form on our Contact page asks for:
- Your name and phone number.
- Your email address, if you choose to give one.
- Your city.
- The type of care you are asking about.
- A message in your own words about what is happening and how soon you need help.
We also keep the date and time of the request and a few technical details that help us tell real requests from spam, such as how quickly the form was filled in and a scrambled (hashed) version of your internet address that limits repeated submissions.
Please do not put Social Security numbers, bank details, or detailed medical records in the message box. We do not need them to schedule an assessment. If care is urgent, call us at (636) 378-7455 instead of waiting on a reply.
What we collect through the job application
The caregiver application on our Careers page has eight steps. Together they collect:
- Identity and contact details: your full name, phone, email, physical address, city, state, and ZIP code.
- Eligibility answers: whether you meet the listed qualifications, whether you have lived in Missouri for the past five years (and in which other states if not), whether you smoke, how you heard about the position, and whether anything would keep you from performing the job duties.
- A background disclosure: whether you have been charged with an offense other than a minor traffic violation, and any convictions, findings of guilt, or pleas of guilt you choose to disclose.
- Family Care Safety Registry status: whether you are registered with the Missouri Family Care Safety Registry and whether you have applied for a Good Cause Waiver.
- Driver's license and transportation: whether you hold a driver's license and since when, whether you hold a skilled license and of what type, and whether you have reliable transportation.
- Experience and availability: whether you have worked with people with physical or cognitive disabilities, the times you are available, and whom you prefer to work with.
- Employment history: up to three past employers, with the company, your position, your supervisor's name and phone number, dates, duties, whether we may contact them, whether you are eligible for rehire, and your reason for leaving.
- References: three people, with their names, relationships to you, phone numbers, and addresses.
- Policy acknowledgements and signature: confirmations that you read our timesheet, drug-free workplace, and qualification policies, your certification and consent, your typed name, and the date.
We do not collect your Social Security number online. Please do not type it into any box on the application. Background screening happens through the Missouri Family Care Safety Registry process after an interview, and only with your written permission.
The application asks for details about other people, such as references and past supervisors. Please list only people who are willing to be contacted.
Information collected automatically
Like most websites, ours receives some technical information whenever a page is requested: your internet address, browser and device type, the pages you ask for, the date and time, and the page that sent you here. Cloudflare, which delivers the site, keeps edge logs of these requests to keep the site fast and secure.
We use Google Analytics 4 to see which pages are visited and how people find us. Google collects measurement data such as pages viewed, device and browser type, and approximate location, and it anonymizes IP addresses in Google Analytics 4. Analytics may set first-party measurement cookies so that one visit can be told apart from another. You can block these in your browser settings or with Google's opt-out browser add-on.
Our site sets no advertising cookies. We do not run ads, we do not use remarketing, and we do not build profiles of you across other websites.
The Contact page includes an embedded Google Map. When the map loads, Google may receive your internet address and use its own cookies, under Google's Privacy Policy.
How we use your information
- To answer assessment requests, schedule visits, and plan care.
- To review caregiver applications, verify references, and complete eligibility and screening steps.
- To send you a confirmation that we received what you sent, and to reply to you.
- To keep the site secure and to prevent spam and abuse.
- To understand how the site is used so we can improve it.
- To meet legal and regulatory requirements that apply to us.
Your phone number and email address are used to respond to your request or application. We do not send marketing text messages, we do not make automated calls, we do not run a newsletter, and the forms do not sign you up for anything. The site has no accounts and takes no payments.
Who we share it with
We do not sell your information, and we do not share it for advertising. We share it only with these groups, and only as needed:
- Launch Automation, the web agency that built and runs this site. It acts for us as a service provider for hosting and delivery. If the email that notifies us of a submission fails to send, a copy of that notification goes to the agency so that nothing is lost.
- Cloudflare, Inc., which hosts the site, stores submissions in its database, and delivers our email.
- Google, for analytics and for the embedded map.
- People involved in your application, such as the references and past employers you list, who may be contacted during hiring, and the state registry that runs the Family Care Safety Registry screening with your written permission.
- Authorities, when the law requires it, or when we need to protect the safety of a person.
How long we keep it
- Assessment requests are kept while we work on your inquiry, and afterward in the agency's records.
- Job applications are kept for the period Missouri employment-record rules require, and are then deleted.
- Server logs and analytics data are kept by Cloudflare and Google under their own schedules and the retention settings we choose.
You can ask us to delete your information at any time. Where the law requires us to keep a record, we keep only what the law requires.
Your choices and rights
You can ask us, by email or by phone, to:
- Tell you what information we hold about you and give you a copy.
- Correct anything that is wrong or out of date.
- Delete your information.
We may need to confirm who you are before we act on a request. You can also block analytics cookies in your browser, as described above. Our site does not respond to Do Not Track signals, because there is no common standard for them, but we do not track you across other websites either.
The confirmation email you receive after a form is a one-time reply to what you sent. It does not add you to a mailing list.
Children
This site is not directed at children under 13, and we do not knowingly collect information from children under 13 through our forms. The pediatric care information on the site is written for parents and guardians. If you believe a child under 13 has sent us information, contact us and we will delete it.
Security
The site is delivered over an encrypted (HTTPS) connection, and submissions travel to our database encrypted. Access to stored submissions is limited to the owner of Love at Home and to the web agency that maintains the site, which acts only on our instructions.
No method of sending or storing information is completely secure, so we cannot promise absolute security. In a medical emergency, call 911, not the form.
Changes to this policy
We may update this policy from time to time. The "Last updated" date at the top of the page shows the latest version, and the new version applies from the day it is posted.
